Shift Schedule: Friday-Sunday, 24 hours/week

Job Overview: As a part-time RN Care Manager I-IP-WKND at Mercy, you will be a vital member of our Care Management team. Your role includes facilitating the discharge planning process from admission to discharge in collaboration with the healthcare team, ensuring quality patient care, patient safety, and effective transition planning. You will promote a seamless care model, coordinate patient throughput, and encourage team collaboration and coordination.

Key Responsibilities:

• Facilitate discharge planning from admission to discharge.
• Promote a seamless care model and ensure patient safety.
• Collaborate with the healthcare team to optimize performance.
• Address complex or high-risk psychosocial needs of patients.
• Perform duties consistent with Mercy’s mission, values, and Service Standards.

Qualifications:

• Education: Graduate of an accredited School of Nursing required.
• Licensure: Current Nursing License in the state of employment required.
• Experience: Preferred 2-3 years in an acute care hospital setting; Care Management or Utilization Management experience preferred.
• Certification/Registration: BLS (CPR) required at hire or within 90 days; Certification in Case Management preferred.
• Skills & Knowledge: Strong communication, assessment, time management, and critical thinking skills. Ability to interact effectively with various levels of the organization. Knowledge of InterQual or Milliman Care Guidelines preferred.
