Job Overview: As an RN Care Manager II-IP, you will be a vital part of our Care Management team, ensuring quality patient care and patient safety. You will facilitate the discharge planning process from admission to discharge in collaboration with the healthcare team, promoting a seamless care model. This role involves effective transition planning, length of stay oversight, and addressing complex or high-risk psychosocial needs.

Key Responsibilities:

  • Facilitate discharge planning from admission to discharge.
  • Promote a seamless care model and ensure patient safety.
  • Collaborate with the healthcare team to optimize performance.
  • Address complex or high-risk psychosocial needs of patients.
  • Perform duties consistent with Mercy’s mission, values, and Service Standards.

Qualifications:

  • Education: Bachelor’s Degree in Nursing (BSN) required.
  • Licensure: Current Nursing License in the state of employment required.
  • Experience: Preferred 2-3 years in an acute care hospital setting; Care Management or Utilization Management experience preferred.
  • Certification/Registration: BLS (CPR) required at hire or within 90 days; Certification in Case Management preferred.
  • Skills & Knowledge: Strong communication, assessment, time management, and critical thinking skills. Ability to interact effectively with various levels of the organization. Knowledge of InterQual or Milliman Care Guidelines preferred.
