Position Overview:
We are seeking a compassionate Registered Nurse to join our home care team, offering a unique opportunity to provide care in the comforting environment of our clients' homes in International Falls, MN. This role allows for close interaction with patients and their families, enhancing the personalized care experience.

What You Will Do:

• Deliver individualized nursing care following the nursing process of assessment, diagnosis, planning, implementation, and evaluation.
• Work collaboratively with a multidisciplinary team to ensure effective patient care delivery and achievement of desired patient outcomes.
• Engage deeply with patients throughout all phases of care, from preventive services to acute treatment, in their own homes.

Ideal Candidate:

• Bachelor’s degree in Nursing required.
• Current RN licensure in Minnesota or a Nurse Licensure Compact (NLC) state.
• Experience in home health care or a related field is preferred, with a strong focus on patient and family engagement.
